Errata

The following errors have been corrected in the version available on this homepage, and therefore only concerns the printed version of the thesis:

• page 61, corollary 4.1.10, missing a precondition:
  "...a set of formulae Δ, such that M |= Δ, and a formula A..."
• page 101, lemma 6.3.3, missing a prime:
  "...there is a derivation Π : Δ |- Δ' for some Δ' such that B ∈ Δ'."
